      <description><![CDATA[<p><b>Job Title</b>:<span>&nbsp; &nbsp;</span>Coordinated Intake &amp; Referral Community Health Worker&nbsp;</p><p><b>Work Location</b>:&nbsp;&nbsp;<span> </span>Community Programs Office</p><p><b>Supervisor</b>:&nbsp; Director, CI&amp;R&nbsp;<br></p><p><b>Employment Type</b>:&nbsp; Full-time</p><p>&nbsp;<b>Employment Classification</b>:&nbsp; Non-exempt</p><p><br></p><p><u><b>Essential Duties &amp; Responsibilities</b></u>: The Community Health Worker (CHW) serves as an advocate or liaison in the community setting primarily in the 32208, 32209, 32210, 32211 and 32277 zip codes. Contacts pregnant women, preconception women and families of children under the age of three to assess risk factors that may lead to poor pregnancy outcomes and/or poor developmental outcomes. The position works in partnership with the community, client or family in providing information and linkage to programs and resources to address risk factors identified by a risk screen or referral. The CHW will serve a critical role in connecting with high-risk families of newborns to home visiting services and supports.</p><p><b>Specific Activities:</b><br></p><p><u><b>Coordinated Intake &amp; Referral</b></u></p><ul><li>Networks with community providers to promote CI&amp;R Referral and/or recruit eligible participants to needed&nbsp; services</li><li>Conduct outreach in the community to engage and recruit women/families for CI&amp;R</li><li> Distributing flyers, brochures, or other informational or educational documents to inform members of the targeted communities.</li><li>Attend Community meetings or health fairs to understand community issues and build relationships with community members</li><li>Assist families to navigate home visiting, healthcare and social service systems </li><li>Conduct Initial Intakes, including attempts to contact, to assess clients' service needs.</li><li>Travel to&nbsp; contact high-risk clients in the service area as assigned when unable to locate </li><li>Refer families to the appropriate home visiting service provider and follow up&nbsp; to ensure referrals are received by service provider and services are provided to families</li><li>Provide immediate referrals to community resources for families with emergent needs</li><li>Provide relevant information to clients about breastfeeding, health care options, parenting education, preventative health education and child development.</li><li>Maintain participant records in accordance with program and funding source requirements</li><li> Develops a weekly calendar of outreach activities and location.</li><li>Conducts formal presentations in the community on CI&amp;R Services </li><li>Accurate and timely data entry of participant information into the Well Family System.</li><li>Locate and research new services and resources for families </li><li>Participate in continuous quality improvement activities</li><li>Maintain confidentiality of children, families and other records and information</li><li> Participate in trainings, meetings, workshops, conferences, webinars, and conference calls as necessary</li></ul><p><u><b>Other:</b></u></p><ul><li>All other duties as assigned by the CHW Team Lead and Director of CI&amp;R.</li></ul><p><u><b>Education and/or Work Experience Requirements:</b></u><br></p><ul><li>High school diploma with two years of experience in the community. Strong customer service skills, experience with social services or public health/maternal child health field preferred.</li><li>Experience working with diverse communities, organizations and agencies preferred. Strong communication skills are required. </li><li>Resident or prior residence of the targeted zip codes of 32208,09,10 is a plus</li><li>Ability to speak Spanish is a plus.&nbsp;</li></ul><p><b><u>Additional Eligibility Qualifications</u>:</b><br></p><ul><li>Excellent verbal and written communication skills, and the ability to work effectively with a wide range of constituencies in a diverse community</li><li>Ability to build trust</li><li> Excellent computer proficiency (MS Office â€“ Word, Excel and Outlook)</li><li>Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.</li><li>Ability to work flexible hours; including evenings, weekends as needed.</li><li> Ability to establish and maintain effective working relationships with Coalition partner agency staff and the public.</li><li>Works collaboratively and effectively within a team</li><li>Must be trustworthy, empathetic, persistent,&nbsp; resourceful and willing to help others </li><li>Knowledge of community agencies and resources.</li><li>Ability to connect with and interact effectively with culturally diverse families and organizations </li><li>Ability to work with families in a non-judgmental and respectful manner</li><li>Ability to communicate effectively with multiple community partners and medical providers</li><li> Ability to use good judgment and make independent decisions with clear and professional boundaries</li><li>Ability to work independently.</li><li> Must be able to commit to the Coalition vision and be passionate about its mission.</li><li>Requires reliable transportation, as travel is required, valid driver's license, good driving record and automobile insurance.</li></ul><p><u><b>Physical Requirements</b></u>:<br></p><ul><li>Ability to safely and successfully perform the essential job functions consistent with the ADA, and other federal, state and local standards.</li><li> Must be able to travel to alternate locations.</li><li>Required to sit, stand, walk, speak and hear while performing the duties of this job, regularly.       <description><![CDATA[<p>Job Title:&nbsp;Case Manager<br></p><p>Work Location:&nbsp;Magnolia Project West</p><p>Division/Department:&nbsp;Magnolia Project</p><p>Supervisor:&nbsp;Team Lead</p><p>Employment Type</p><p>â˜’&nbsp;Full-time</p><p>â˜&nbsp;Semi-full time</p><p>â˜&nbsp;Part-time</p><p>Employment Classification</p><p>â˜&nbsp;Exempt</p><p>â˜’&nbsp;Non-exempt</p><p>&nbsp;</p><p>Essential Duties &amp; Responsibilities:&nbsp;Coordinate preconceptional and interconceptional services for prenatal, non-pregnant high-risk women and their partners.</p><p>&nbsp;Specific Activities:</p><p><span>Case Management</span></p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Coordinate prenatal, pre- and interconception services for high-risk women.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Utilize selected standardized curriculum and intervention and document activities.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Assess risk and develop risk reduction goals/activities and link participant to services.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Ensure that a comprehensive Life Course Plan is developed for each participant</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Link male partners of enrolled women to group activities and individual services.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Assist with and ensure participation in scheduled the Life course group activities.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Provide services in participantâ€™s home, or current living location.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Provide social and mental health linkages and support.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Follow up with all providers involved with the family.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Provide feedback to all agencies involved.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Participant in case conference as required.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Complete and maintain all documentation, ensuring confidentiality</p><p>&nbsp;Other:</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;All other duties as assigned by the Team Lead or Magnolia Project Director.</p><p>Education and/or Work Experience Requirements:</p><p>Â·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Two years experience as a case manager</p><p>Bachelor degree in the field of maternal child health or social science preferred.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Will substitute 5 years professional experience in direct service, social services, maternal and child health or related field.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Experience working with diverse communities, organizations and agencies preferred.</p><p>&nbsp;</p><p>Additional Eligibility Qualifications:</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Knowledge of local health and human service providers</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Ability to work flexible traditional and non-traditional hours</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Ability to be a self starter and work independently as necessary</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Good communication oral and written</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Maintain professional boundaries</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Ability to work independently and to carry out assignments to completion within parameters of instructions given prescribed routines, and standard accepted practices.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Must be able to communicate effectively and be a team player at the Coalition.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Must be able to commit to the Coalition vision and be passionate about its mission.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Requires reliable transportation as travel is required, valid driverâ€™s license, good driving record and automobile insurance.</p><p><br></p><p>Physical Requirements:</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Ability to safely and successfully perform the essential job functions consistent with the ADA, and other federal, state and local standards.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Must be able to travel to alternate locations.</p><p>Â·&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Required to sit, stand, walk, speak and hear while performing the duties of this job, regularly.
